NOTES ON LEAGUE AFFILIATION

Submission of this form and payment are required to complete your league affiliation.

League affiliation is the only way to make EPA events such as Intercounty, Interleague and Champion of Champions available to the players in your league.

If your league operates twice a year (i.e. Summer and Winter leagues), then you must submit an affiliation and pay for each league, and specify in the league name field which league each form is for.

Payment details are:

£5 per league venue, capped at £50
